Philippine Airlines Flight 421 was a scheduled passenger flight from Tokyo's Haneda Airport to Manila Ninoy Aquino International Airport which veered off the runway at Tokyo during its take-off phase.

Accident

At approximately 15:22 on April 18, 1977, Philippine Airlines Flight 421 ran off runway 15L at Tokyo's Haneda Airport during its take-off phase. The aircraft banked to the left and touched down again, veered off the runway and continued for 800 meters off the runway before coming to a stop. There were no fatalities among the aircraft's crew and passengers. The aircraft, a McDonnell Douglas DC-8-53 with tail number RP-C803 was destroyed during the incident and subsequently retired from service (written off).

According to final reports of the accident, the probable cause for this accident was due to a faulty Left elevator geared tab drive arm and elevator gust lock crank which failed during the take-off phase of the plane.
